Check uses a consistent format for most Tax Documents. This includes all of the information needed for an employer to understand their filing in a cleaner and more consistent layout, which will help reduce common questions about tax filings

Is the Tax Document what Check files?

Most agencies accept bulk electronic files for reporting. The Tax Documents we produce for the employers lay out the most important information for their review and tracking. We do not file this exact version of the Tax Document with the agencies. In Tax Year 2025, we changed the format of the employer copies of many Tax Documents. The information we send to the agency on your employer’s behalf is still in the same format. This change only impacted the format of what we share with employers.

Is my filing accurate?

Yes, we file a correctly formatted electronic or paper form with the agency on your employer’s behalf. You do not need to do anything with this Tax Document.

What information is included on the Tax Document?

We include the most important information that the employer needs to understand their filing. This always include the agency, filing name, filing period, and filing due date. Depending on the information that the agency requires it may also include gross wages, taxable wages, rates, withholding amounts, employee counts, time period breakdown, and employee breakdowns. Only information that is in the original filing will be included in the employer copy, so each agency may have different information. When information is not required by the agency, it will be flagged as β€œNot required by agency”.

What does the template look like?

Here are example screenshots of the template. Only the information that is sent in the submission filing will be included, so the exact file for each agency will include a subset of this information.

How do we put our logo on the template?

Check will use the logo provided by the partner for use in components. If no logo is found, then we will leave that section blank. We will never include Check’s logo. If you want to update that logo, please share an updated logo PNG with us.

Why is the format different than the official state form?

We send most agencies an electronic file of your tax data. There is no requirement for how we present this information to you. Therefore we publish a clean, consistent format that helps your employer more easily find and understand the key information in the filing.

Why is some information missing from the official agency form?

This document includes all of the most important filing information that you should need. It leaves out uncommon information that agencies use to label or filter your filing for their internal purposes (i.e. employee industry codes). This information is usually not needed by the employer. If for some reason, you need this information, please contact customer support.

Are agencies okay with this change?

Yes, we confirmed with every agency to determine whether or not they regulate the employer copies of forms. For those that do regulate this form version, we use the agency approved versions.

What agencies use the standard Tax Document format?

Every agency besides the below will have the Tax Documents provided in this format.

Should I send this to the tax agency?

No, this is not a filing. This is for informational purposes only and should not be sent to the agency. The information matches what was filed with the agency, so the employer can use it to answer questions that the agency asks.

Where can we view these Tax Documents?

You can view these forms through the Filings API and in Console. There is no change to how or when the tax documents are created and shared.

What if I need information that isn’t on this form or the official agency format?

Contact customer support and we will work to resolve your employer’s issue.
